I had one of these at my other house; it didn't last. I came across a 4-pack at the garden store in late spring and planted those here. They just don't seem to be growing well. Not a lot of new leaves, and very few flowers. The soil, sun and watering conditions all seem to be appropriate from what I read online. Any thoughts? I generally have a green thumb.

I never had a problem with geums but we have a wild variety here that's all over. Since we have them wild, I suspect they are ok with clay soil, not too much water and acid soil as that is what is here. Also not too much heT here but where the wild ones are they get a lot of sun.

How long have you had them? I know that they are generally considered a short-lived perennial which requires dividing fairly often in order to keep them going.
I transplanted some wild native "prairie smoke" geums into my garden earlier this year and they took awhile to get started- I wasn't sure that they were going to make it for awhile, but they finally perked up and look okay now.
Any signs of disease on them?
